We accept all commercial payor insurances including Medicare.

An explanation of benefits (EOB) is a document that is sent by your insurance provider that explains how your claim was processed. It indicates what was paid and/or what was denied along with basic account information such as allowed charges, co-pays, covered and non-covered tests, and explanation of payments.

It is not a bill, nor does it represent final patient responsibility in some cases.

You do not need to pay anything until you receive a bill from us. Our policy is to only send a single bill in the mail. We will never send a second bill.

You may have received a bill due to one of the following reasons:

• Your insurance policy requires that it is your responsibility for a co-pay or deductible charge.
• Your policy may not cover certain laboratory tests.
• Your policy has maximum benefit limitations.
• Information regarding your group number, policy number, diagnosis code, or other required information to process your claim was not provided to Precision Health Solutions or was provided incorrectly.
• Your insurance company needs information from you as stated on your explanation of benefits (EOB).
• We were instructed by your physician to bill you directly.

If you receive payment directly from your insurance company for Precision Health Solutions testing, please endorse check with your name and the statement “Endorsed over to Precision Health Solutions” along with your EOB.

Please note, our policy is to work with you on a personal level and want to be sensitive to your situation.  Please contact us directly to discuss any financial hardship payment options you may be eligible to receive.

PHS testing is paid for by most insurance companies, including Medicare and Medicaid. After your testing, we will submit a claim to your insurance company whether you are in-network or out-of-network. Your health insurance company may send you an Explanation of Benefits (EOB) which is NOT a bill and does not require payment.

We will work with your insurance company to ensure the maximum benefit is applied to our services. If your insurance company requires a balance to be paid, you will receive an invoice from Precision Health Solutions. Please call us at 727-235-0886 to discuss payment options.

PHS offers a very reasonable, discounted patient price for uninsured patients to ensure that all patients can have access to these important diagnostic tests.

Our pricing is $125 per test with 24-48 hour results. For results guaranteed next day, we charge $250.
